Is realizing 3+ months late that there was an additional form that should have been filled out and filed with the IRS.

At least it doesn't change my tax amount at all, but still, IRS forms? Talking to the IRS? Ulp.

When I've had to talk to them, they have been bureaucratic and rigid, but at least vaguely helpful within the bounds of what they are allowed to do, and some of them were even friendly.

Alternatively, assuming nothing you did send in made explicit reference to the presence or absence of this form, you could just wait for them to ask, and then send them a duplicate copy of the form which was sent originally and must have been lost due to a clerical error.

Same here. Also, my experience forgetting theoretically-required forms that don't actually chance my tax burden is that they didn't actually care.

Me, I'd just wait and see if they asked for it, though I can see how you'd want to get it squared away.
